Reminder for all WCCEAL Members:

The 2024 1^st Quarter Quality Improvement Variables data submission period is OPEN and will remain open through April 15^th. Please make sure to get your QI Variable data submitted as soon as you can!

The annual Satisfaction Surveys must also be returned by April 15^th. If you haven’t done so already, please generate your surveys and distribute to your residents. The survey responses can be mailed to the address below or scanned and emailed to wcceal@qid.wisc.edu (please include ALC name and license number in the file name).

• Click Here: https://qid.wisc.edu/login/index.php  and use your login credentials
• Click the eQuality Tab
• Click the QI Variables Tab
• Click QI Variables Form gray button. (The photo below shows where to locate the form)

How to Generate the Annual Satisfaction Survey:

• Click Here: https://qid.wisc.edu/login/index.php and use your login credentials
• Click the eQuality Tab at the top center of website
• Click the Surveys Tab
• Click the Go To Satisfaction Survey Generation button
• Select if you would like to generate a 1 page or 2 page version
• Click Save and Generate Survey PDF
