The Division of Quality Assurance (DQA) is planning to host RAI/MDS 101 training for new MDS coordinators. They are asking you to complete and submit the DQA Minimum Data Set (MDS) Training Research Survey to help them determine what training topics would be most beneficial to your MDS coordinators as well the best location and time to hold the training.

Please complete and submit the DQA Minimum Data Set (MDS) Training Research Survey before April 18, 2022.
